Payment Instructions
Full payment is due at the time of service unless previous arrangements have been made. We accept the following methods of payment:
We offer a complementary service of filing insurance forms for patients who have insurance. Although we are able to bill your insurance for you, the patient portion will be due at the time of service, and will be estimated based on your current insurance status. Financing: Our office will be glad to provide you with the names of companies which specialize in financing your comprehensive dental treatment. Insurance: We accept all PPO insurance plans, but are not contracted directly with any insurance company. We are considered an out of network provider. We will still file all claims for you at the time of service. This prevents us from being restricted on the type of care we can provide our patients. Remember, your insurance plan is designed solely on the basis of profit for the insurance company. The premium minus benefits paid equal profit for the insurance company. Most insurance companies are not in the business of patient care and do not have your best interest in mind. If you have questions about your benefits or coverage, then you should contact your employer or the insurance company. We are available to help you with insurance as much as we can. Only Dr. Nioofar Hubrich is a MEDICAID provider. |
